than 120 hours prior to scheduled hearing - $400 if cancelled less than two
weeks prior to scheduled hearing
Travel - $.34 per mile plus actual
lodging and meal expenses. Actual air fare. Travel agent service fee and change
fee for changed non-refundable tickets.
Docketing Charge - $30
NOTE: The
arbitrator shall not charge any fee other than the above stated fees.
